In 2019, Neeraj faced a major setback when he suffered a shoulder injury, which required surgery. This forced him to take a break from the sport, and his participation in the Tokyo 2020 Olympics seemed uncertain. However, with rigorous rehabilitation and an indomitable spirit, Neeraj made a remarkable recovery and returned stronger than ever.

He followed this with a gold medal at the Diamond League Finals in Zurich, further solidifying his status as one of the best javelin throwers in the world.

In 2023, Neeraj won the gold medal at the World Athletics Championships in Budapest, becoming the first Indian to achieve this feat. His consistent performances have made him a force to be reckoned with in international athletics.

In 2024 Paris Olympic, he secured a silver medal for India with a throw of 89.45 m and was competing with Pakistan's Arshad Nadeem, who secured gold with a throw of 92.97 m.
